Step 01

Plug in Home Hub

Take your Home Hub out of the box and place it in a room that you spend a lot of time in - your bedroom, living room or kitchen are ideal! Make sure your Home Hub is ALWAYS plugged into a power source.

Wi-Fi: For best results, connect your Home Hub to your Wi-Fi network by following these easy steps: on the bottom right corner of the Home Hub screen click on settings, then select Wi-Fi settings, choose your Wi-Fi network, and enter your Wi-Fi password.

Gather Undergarments

Gather 6 of your CLEAN underwear, pajamas or bras. Select the pieces of clothing you wear most frequently. Your clothing should fit snug against your body.

Step 03

Open Health Tag

Peel the teal-colored backing to remove one Health Tag from its packaging. Then peel the white liner to expose the adhesive.

Step 04

Attach Health Tag

Underwear/PJs: Adhere 1 inch from center on the front inside of your waistband and 0.5 inches from the top.

Bra: Adhere to inside wing near the cup.

Note: The Health Tag should fit snug against your body. Do not remove Health Tag once attached - each Health Tag stays on one piece of clothing for up to a year.

Step 05

Press and Hold

Press Health Tag firmly into clothing, massaging for a few seconds to ensure placement. Wait to wear or wash clothing for 24 hours. This helps the Health Tags stay attached.

Repeat Steps 3 - 5 for each Health Tag

Step 06

Wear Daily

Congratulations, you’re set up! Remember to wear your Health Tag daily. You can view your activity by checking your Home Hub.

Questions You May Have

How do Health Tags work?

– Health Tags collect health related data daily. This data will then be sent to your Hub.

– Your Hub will collect and transmit your health data to your care team.

– Your care team is able to monitor changes in your health.

How Do I Wear Health Tags?

– Make sure the soft suede side is in contact with your skin

– Adhere Health Tags to clothes, NEVER to skin

– Only wear one Health Tag at a time

– Make sure your waistband does not roll over

What is My Health Tag Monitoring?

Your Health Tag contains sensors which automatically monitor breathing, pulse rate & activity. You will see activity data on your Home Hub and all clinical detail will go directly to your doctor.

How do I attach a Health Tag?

Health Tags are attached to the inside of your underwear, bra, or pajamas. The suede side should always be touching your skin and fit snuggly when worn. Only one Health Tag should be worn at a time.

What happens if my Health Tag falls off?

Your Health Tag shipped with several replacement adhesives. To reattach, peel the one side of the backing. Both sides are identical, so either side is OK. Attach the re- placement adhesive to the back of the Health Tag (over the old adhesive is OK) and apply pressure on it for 5 secs with your thumbs. Remove the second backing to ex- pose the other side of the adhesive. Then attach the Health Tag to your clothing as usual. Do not wear or wash the garment for the next 24 hours to help with adhesion.

Do Health Tags work when I am away from home?

Your Health Tags will store data for up to 24 hours when you’re not near your Home Hub. If you plan to be away from home for more than 24 hours, you can bring your Home Hub with you or resume your monitoring once you return.

How do I care for my Health Tags?

Health Tags are washer and dryer safe and do not need to be charged. Once attached to your clothes, wear your clothes like you normally would and wash/dry on low or medium heat. Do not remove or transfer your Health Tag between clothing.

How frequently is my data being monitored

Health Tags are not a real-time monitoring service; however, when near your Home Hub your data will be continuously sent to your care team. The frequency at which they review and reach out to discuss this data is up to their discretion. Please contact your provider directly regarding their specific Remote Patient Monitoring policy.
